• Bring your driver's license, or government issued identification card

• Bring your insurance insurance card.

• Fill out the paperwork below before your appointment.

• Please make sure you know the name, dose, and frequency of any medications you are on. If you are on a lot of medication, you may find it easier to print a list of this or bring your medications with you.
